Pawtucket Falls Healthcare Center is looking for a Full-Time Social Worker for long-tearm care unit to join our amazing team.

Qualifications

  • Bachelor’s degree in social work, or bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology; AND 
  • One (1) year of supervised social work experience in a health care setting working directly with individuals 
  • Effective verbal and written English communication skills. 
  • Highest level of professionalism with the ability to maintain confidentiality. 
  • Ability to communicate at all levels of the organization and work well within a team environment in support of company objectives. 
  • Customer service oriented with the ability to work well under pressure. 
  • Strong attention to detail and accuracy, excellent organizational skills with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with a high level of quality and productivity. 
  • Strong analytical and problem-solving skills. 
  • Ability to work with minimal supervision, take initiative, and make independent decisions. 
  • Approachable, flexible, and adaptable to change. 

Responsibilities

- Plans, controls, and directs the services of the Social Service Department.

- Monitors resident progress and collaborates with the Interdisciplinary Team to ensure appropriate and high-quality services to residents.

- Coordinates discharges and ensures residents are appropriately discharged with ancillary services provided and planned.

- Participates in the development and maintenance of resident care plans.

- Is responsible for the timely documentation of initial assessments, interim notes, progress notes, and completion of the MDS.

- Serves as a resident advocate.
